Enrollment Form
The Holland Reflexology Institute will soon be sending enrollment forms out to interested students for the August 17, 2009 class.
- Email the Institute at doughollandjr at gmail.com to request a .pdf enrollment form.
- Registration fee is $50.00 which is applied to the total cost of tuition upon acceptance of enrollment.
- Fill out the .pdf form you received by email and downloaded and mail it, along with the check for $50.00 made out to ‘Doug Holland‘, to:
